I masked the women on the picture, as she possibly has nothing to do with the issue here.

The picture filename suggests it was taken on March 20, 2004, the EXIF information indicate it comes from a FinePix S5500 digital camera, taken on January 1, 2004 (possibly the camera calendar was not set) but the EXIF data also indicate that the image was edited with a picture editing software. The email message contained no information at all, only the picture.
The so-called "strangeness" on the picture is what people call an "orb", that is, nothing unexplained at all. This "orb" being in color upon a black and white picture, it was obviously pasted in to attempt a fraud - a quite amateurish and ill-conceived fraud.
